SKYELETES PARTICIPANT WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in SkyeLetes basketball league activities, I hereby agree as follows:
1. ASSUMPTION OF RISK: I acknowledge that basketball is a physical contact sport and that participation involves inherent risks including, but not limited to, sprains, strains, fractures, concussions, and other injuries. I voluntarily assume all such risks, both known and unknown.
2. RELEASE AND WAIVER: I hereby release, waive, discharge, and covenant not to sue Skyes Over London LC, Solenterprises International Nexus & Holdings LLC, their officers, directors, employees, agents, volunteers, and affiliates from any and all liability arising from my participation in SkyeLetes activities.
3. HEIGHT VERIFICATION: I certify that my stated height is accurate to the best of my knowledge. I understand that my height will be verified barefoot at check-in and that providing false information may result in disqualification without refund.
4. MEDIA RELEASE: I grant SkyeLetes and its affiliates the right to photograph, film, and record me during league activities and to use such media for promotional purposes including social media, websites, and marketing materials.
5. CODE OF CONDUCT: I agree to conduct myself in a sportsmanlike manner at all times. I understand that unsportsmanlike conduct, violence, or harassment may result in immediate removal from the league without refund.
6. REFUND POLICY: I understand that registration fees are non-refundable except in cases of medical emergency (with documentation) or league cancellation. Partial refunds may be issued at the sole discretion of SkyeLetes management.
7. PAYMENT TERMS: By registering, I authorize the charge of the current league induction fee. Champion payouts are subject to tax reporting requirements.
8. GOVERNING LAW: This agreement shall be governed by the laws of the State of Arizona.
I have read this waiver and release of liability, fully understand its terms, and understand that I am giving up substantial rights by signing it. I acknowledge that I am signing this agreement freely and voluntarily.
